About Our Program

We provide health care or supportive care to the client at his or her home by our trained professionals. As our clients have the luxury of receiving the services within the comfort of their own home this is also called as 'Nursing Home without walls'.

Patient Qualification

We provide services to elderly, disabled and other individuals that need in-home care and to those who needs help with personal cares and Activities of Daily Living (ADLs).

You are eligible to receive a home care service if you:

  • Have Medicaid or/and Medicare
  • Requires medically reasonable and necessary intermittent skilled service
  • Are expected to need long-term care service for at least 120 days.
  • Need to be on Medical Assistance
  • Live in a community-based residence
  • Need help with your daily activities such as bathing, grooming, dressing, or participating in the community

Activities of daily living refer to:

  • Bathing
  • Dressing
  • Transferring
  • Using the toilet
  • Assist with Feeding
  • Assist with Walking
  • Light housekeeping
  • Meal Preparation
  • Reminding to take medications
  • Grocery Shopping
HHA Qualification

There are few qualifications to becoming a Home Health Aide (HHA)

  • Must be over 18 years old.
  • Must be legally allowed to work in the United States.
  • Interested individual must be clear of any criminal charges in his/her record (including misdemeanors).

A complete Physical examination (physical form will be provided). Physical exam must include the following:

  • A drug screening (with lab report)
  • Rubella Titre
  • PPD Mantoux (Must have two PPD test)

Each PPD test consists of two parts: Part I – having the test “placed” on the arm and Part II – having the result “read” 48-72 hours later by a healthcare provider. Therefore, to have two PPD tests means that you will need to perform this cycle twice. Again, each test must be performed AT LEAST 7 days apart from each other and BOTH tests need to be performed WITHIN a year of taking this course. Therefore, it will take a minimum of 9-10 days to complete and have a result on two PPD tests.
If you have a positive skin test result you must take a chest X-ray.

What is Home Health Care?
Home Health Care is personal based care provided in the client’s home by our healthcare professionals. People also refer to Home Health Care as ‘Nursing Home without walls’ because our clients have the luxury of receiving health care in the comfort of their own home.

How do I obtain a Home Health Aide License?
Home Health Aides that has completed the training program receives a certificate from New York State. You can receive training from X-treme Care.

How long does it take to become a Home Health Aide?
To complete the entire program, it takes 14 days. HHA trainees will take class for first thirteen days and learn about how to take care of a patient. On the last day, HHA trainee will be paired with an patient along with a field nurse.

Who is eligible to join the Home Health Care Program?
Patient should have Medicaid or/ and Medicare. They need to be on medical assistance and live in a community based residence.

Consumer Directed Personal Assistant Service
About Our Program

The Consumer Directed Personal Assistant Service (CDPAS) is an alternative to traditional home care. The CDPAS is a Medicaid program enabling self-directing individuals, or their designated representative, to assume the responsibilities of their own care. The consumer and/or designated representative is responsible for recruiting, interviewing, hiring, training, supervising, scheduling and termination.

Patient Qualification

Consumers in NYC, Nassau, Suffolk and Westchester through different forms of Medicaid Managed Care. Dual eligible consumers (meaning they have both Medicare and Medicaid) must first enroll in a Managed Long-Term Care (MLTC) plan in order to receive care under the Consumer Directed Personal Assistant Service. Consumers with just Medicare must be enrolled in a mainstream managed Medicaid plan. In both cases, the Consumer Directed Personal Assistant Service is administered by the plan.

PA Qualification

There are few qualifications to becoming a Personal Assistant (PA)

  • Must be over 18 years old.
  • Must be legally allowed to work in the United States.

Additional Requirements

Patient and PA cannot live in same household
Child can become a PA for his/her parents but parent cannot become a PA for his/her child. Spouse cannot be each other’s PAs.
